Roster – this is the Roster that you will find on the website – filled out with the directions I sent in the other email – unless that is not correct.

Physical and Birth Certificate (if neccessary) on left and Application with player image on right – players pages should be listed in the same order as they are on the Roster. Each player will have a fold out like shown in this picture with their physical form on the left side and their application on the right side.
